#AskPracMed – What do we do if someone has a serious sprain on site?

First, make sure it is actually a sprain and not a fracture — the two can be difficult to distinguish without imaging, and different people tolerate pain very differently. Once you’ve determined it’s a sprain, the RICE method is no longer the recommended approach. Rest does not engage the lymphatic system, which means swelling stays put rather than clearing. Current evidence supports continued movement within the person’s pain tolerance, which keeps the lymphatic system active and supports recovery. For full guidance, look up the PEACE & LOVE protocol, or consult a physio. Don’t ice it and sit them down for the rest of the day.
